Customized Bag Order Form

Please fill out the form below to place your order. Ensure all fields are completed accurately so we can prepare your order for pickup.

Pick-Up:

Bag orders may be picked up Monday-Thursday during our open hours 10am-2pm.

Bag Contents:

Every bag includes: at least 1 complete outfit of clothing, pajamas, socks, underwear, seasonally appropriate outerwear, age-appropriate toiletries, toys, books, school supplies or activity kit, a handmade quilt or fleece blanket and a stuffed animal. 

Who Needs to Pick Up the Bag:

A representative from the agency must be the individual to pick up the bag order(s). Due to privacy concerns, we cannot distribute the bags to the recipients directly. 

Processing Time:

We aim to process and prepare your order within 1-2 days of form submission. For larger orders (25+), processing time is 2-3 days. In November and December, processing time may be up to 1 week. Note: If this is an emergency order, please call us or indicate this in the “special instructions” section.
